 Name of the template page
Select the option "Page name" and choose a template page from the drop down menu to apply that template to the active page. Uncheck the option will remove the template page from the active page.

Press the "OK" button to accept the settings.

Press the "Cancel" button to discard any changes and exit the dialog.
